A message from the Nurse
Welcome to North Scott High School! In order to participate in athletics at the high school level, students need an Iowa Athletic Pre-participation Physical Examination Form completed each year. The physical is good for 12 months with a 30 day grace period. Please schedule your appointment early in the summer to avoid the August rush. Download the form below and take it to your doctor's appointment with you.
Concussion Baseline Testing
Incoming freshmen are required to have a Baseline Concussion Test completed if they are participating in any contact sport. Contact sports include: football, volleyball, basketball, wrestling, soccer, baseball, softball, cheer, and dance. Excluded sports are: cross country, golf, bowling, tennis, track, and trapshooting. Genesis Physical Therapy offers free testing.
Certificate of Dental Screening
Incoming freshmen are required to have a Certificate of Dental Screening or a Certificate of Dental Screening Exemption on file with the High School Nurse, Wendy Peterson. See below for an informational letter and screening forms.

12th Grade Immunization
All students entering 12th grade are required to have 2 doses of the Meningococcal Conjugate vaccine prior to the first day of school in August (2024), unless the student has a Certificate of Immunization Exemption on file. Learn more in the informational letter below.
